The parry mechanic allows players to counterattack incoming melee attacks, but the knife has limited durability and can break. It's possible to beat the game using only knives. There's a dog in the beginning of the game that Leon has the choice to rescue. Some speculate that the dog may be infected and fought later in the game. Injured ganados will rise again stronger if not immediately executed on the ground. The game will keep all the brutal deaths from the original, including new ones. Ashley's voice actor has been improved, and players can command her to follow or stay further back using the R3 button. However, Ashley cannot stay put and will still move with Leon, putting her in risk of being scooped up by enemies. In addition, the remake will feature side quests in the form of blue flowers posted around the environment. After completing these quests, players must return to the merchant for a reward. There will also be new puzzles that reappear in spirit, and exploring the larger area of the village reveals various new routes and interior options for progression.

The cabin fight sequence will also be similar to the original but with a few tweaks. Players can now barricade windows using nearby planks, and Luis will behave more intelligently and be a more capable ally. The weapon storage case has also been updated, allowing players to equip different cases that grant different perks such as increased ammo drops.
